Why These Forms Matter for Your Safety and Healing
Before your first MASSAAH'G session, you'll be asked to complete intake forms including a service agreement and health history questionnaire. While paperwork might not be the most exciting part of your healing journey, these forms are essential for providing you with safe, effective, and personalized care.
Why Health History Forms Matter
Your body tells a story, and your health history helps me understand that story before we even begin. This information allows me to:
- •Identify Contraindications: Certain conditions require modified techniques or may mean massage isn't appropriate at this time. Your safety is my top priority.
- •Customize Your Session: Knowing about past injuries, surgeries, or chronic conditions helps me tailor the session to your specific needs.
- •Understand Medications: Some medications affect how your body responds to massage. This information helps me adjust pressure and techniques appropriately.
- •Track Your Progress: Over time, your health history becomes a record of your healing journey, helping us celebrate improvements and adjust our approach.
What the Health History Form Includes
Our comprehensive health history form asks about:
- Current health conditions and concerns
- Past surgeries, injuries, or accidents
- Medications and supplements you're taking
- Allergies (including to oils, lotions, or scents)
- Areas of pain, tension, or discomfort
- Pregnancy status (if applicable)
- Cardiovascular conditions
- Skin conditions or sensitivities
- Recent illnesses or infections
- Your goals for the session
The Service Agreement
The service agreement establishes clear expectations and protections for both of us. It covers:
- Scope of services provided
- Your rights as a client
- Confidentiality of your information
- Cancellation and rescheduling policies
- Payment terms and expectations
- Professional boundaries
- Communication preferences
- Consent for treatment
Conditions That Require Special Consideration
Certain conditions don't necessarily prevent massage but require modified approaches:
- • High blood pressure or heart conditions
- • Diabetes
- • Pregnancy
- • Cancer (current or history)
- • Blood clotting disorders
- • Osteoporosis
- • Recent surgery
- • Autoimmune conditions
- • Skin conditions
- • Mental health conditions
Having any of these conditions doesn't mean you can't receive massage. It means we'll work together to ensure your session is safe and beneficial. In some cases, I may request clearance from your healthcare provider.
Keeping Your Information Current
Your health isn't static, and neither should your health history be. Please inform me of any changes to your health status, including:
- New diagnoses or conditions
- Changes in medications
- Recent surgeries or procedures
- New injuries or areas of concern
- Pregnancy
- Changes in your goals or preferences
